How to View Tax Reported Prizes

1. Sign In from the menu. The menu is accessible by selecting the “ ” icon in the upper-left portion of your screen. Select “Sign in” from the mobile menu and enter your Michigan Lottery account credentials.
2. Select “My Account” from the mobile menu.
3. Select “Tax Reported Prizes”.
4. Select “Submit Claim” to start the claim process. Note: certain claims will require you to send additional information; select “VIEW CLAIM POLICY” to learn more.
5. Enter the first (5) digits of your social security number, check each box to sign electronically, then select “SUBMIT”.
6. View confirmation screen.

Desktop Computer

1. Sign In and select your name to open the Account Menu.
2. Select “My Account” from the menu.
3. Select “Tax Reported Prizes”.
4. Select “Submit Claim” to start the claim process. Note: certain claims will require you to send additional information. Select “VIEW CLAIM POLICY” to learn more.
5. Enter the first (5) digits of your social security number, check each box to sign electronically, then select “SUBMIT”.
6. View confirmation screen.
