How do I print a W-2G form for an online prize?

For an individual online prize of $2,000 or more, a W-2G form will be made available online for tax-reporting purposes after the prize claim reaches a status of “Completed.”

To download and print the W-2G form:

1. Sign in to your Michigan Lottery account and visit Pending Prizes History. This will open the Tax Reported Prizes section of your account.

2. Locate the applicable prize with a status of “Completed” and select “View.”

3. On the prize details screen, select “View W-2G Form.”

4. Download the completed W-2G form to your device and print or save it for your records.

Each prize has a separate W-2G form and must be downloaded individually.
