How do lottery clubs claim prizes?
A lottery club is a group of at least two individuals who purchase wagers in a Lottery game together. If the club wins a prize, the prize is divided among the club members. It is up to the lottery club to determine its rules of operation, including how prize money will be distributed to members.
To claim a club or group prize, submit:
- A completed Substitute 5754 form (BSL-A-2164). Review the Instructions for the Substitute 5754 Form before completing the form.
- The original winning ticket signed on the back.
For prize claims of $1,000 and above, the required identification must also be provided for the applicable club members, including a valid government-issued photo ID and Social Security card. The name on the photo ID must match the name on the Social Security card.
