How to claim a prize by mail

Michigan Lottery prizes of $99,999.99 or less can be claimed by mail.

Mail copies of the required documents and identification, as applicable, along with the original winning ticket signed on the back, to:

Required documents and identification

For all individual prize claims:

  • A completed Ticket Receipt Form: English, Spanish, or Arabic.
  • The original winning ticket signed on the back.

For all club or group prize claims:

For all prize claims of $1,000 and above:

  • A photocopy of a valid government-issued photo ID, such as a driver’s license or state ID.
  • A photocopy of a Social Security card. The name must match the name on the photo ID.


Keep a photo or photocopy of the front and back of your signed ticket and your completed Ticket Receipt Form. Ensure all submitted copies are clear and legible.

The winning ticket must pass all validation and security tests of the Bureau and be confirmed by the Bureau as valid before a prize is issued.

Prizes claimed by mail will be paid by check mailed to the address provided on the Ticket Receipt Form. The Lottery reserves the right to limit the number of tickets claimed per day.


Prizes of $1,000 or more

If you win a prize of $1,000 or more, state law requires the Michigan Lottery to withhold any outstanding debts on record with the State of Michigan, up to and including the full amount of the prize. Questions regarding such debts should be directed to the Michigan Department of Treasury at 517-636-5265.


Tax information

Michigan Lottery prizes may be subject to federal, state, and local income taxes. You may be required to make estimated tax payments by filing Form 1040ES with the Internal Revenue Service.

If you have questions regarding income tax, contact the Internal Revenue Service at 800-829-1040, the Michigan Department of Treasury at 517-636-5265, your local taxing authority, or a tax professional.

If you claim an individual prize of $2,000 or more, you will receive a W-2G form for your yearly tax return. To request a duplicate tax form for an in-store ticket prize, call the Michigan Lottery at 517-373-6001. Lottery Claim Centers cannot reprint tax forms.


Tax withholding by prize amount

Prizes of $1,999.99 and below: The Lottery does not withhold any taxes.

Prizes of $2,000 through $5,000: The Lottery does not withhold any taxes but is required to report the winnings to the Internal Revenue Service and the Michigan Department of Treasury. The winner must report the income in the tax year it is claimed.

Prizes greater than $5,000: For U.S. citizens and U.S. resident aliens, the Lottery must withhold an estimated 24% federal tax and 4.25% state tax.

Non-Michigan U.S. residents: For prizes over $5,000, the Lottery must withhold 24% federal tax and 4.25% state tax, regardless of the winner’s state of residence.

Non-U.S. residents: For any Michigan Lottery prize, the required tax withholdings are typically 30% federal tax and 4.25% state tax. Non-resident alien winners claiming individual prizes will receive a 1042-S form at the time of prize payment for use in completing their tax return.

Tax withholdings by the Michigan Lottery do not necessarily satisfy a winner’s total tax liability. Winners should contact the Internal Revenue Service, the Michigan Department of Treasury, and a tax professional to confirm required tax payments.
