Acceptable Documents

Document Requirements: 

• All copies must be clear and in color
• Verification documents (affidavit, letter of consent, Direct Deposit Form, etc.) cannot be handwritten. 
• Only account change requests (email change, home address change, name change) canbe handwritten. 
• 'Proof' must be on the same document.
   

Photo ID

• Driver’s license (any state) with expiration date.
  1. Temporary paper ID is acceptable when submitted with old photo DL and visible new expiration date.
• State issued photo identification card with expiration date.
• Carry-concealed weapons (CCW) license with photo and expiration date.
• Military ID with photo and expiration date.
• US Passport with photo and expiration date.
• Passport Card with Photo and expiration date.
• Permanent resident card with photo andexpiration date.
• Veterans Health Identification Card withPhoto and Expiration Date.
• Tribal ID.
  
  

Proof of Home Residence/Proof of Address (POA)

• Valid photo ID with the new address (see above)
• Utility bill or credit card bill issued within the last 90 day
• Account statement from a financial institution issued within the last 90 days
• Pay stub or earnings statement issued with the name and address of the employer and employee issued within the last 90 days
• Proof of auto insurance issued within the last 6 months
• Auto registration issued within the last 12 months
• The proof of residency must contain the player’s name, address, and date of issue
  
  

Proof of Social Security (SSN)

• Social Security Card
  
  

Proof of ACH

• ACH documentation must be on a single typed document with all below elements:
  1.  Full player name
  2.  Name of financial institution
  3.  Full routing number
  4.  Full account number(CU member ID unacceptable)
• A bank letter (e.g. direct deposit letter) or voided check are examples of acceptable proof of ACH
• Deposit slips are unacceptable
• The financial institution must accept ACH bank transfers

Proof of Card Ownership

• Card belongs to player:
  1. Copy of card in question
  2. Copy of player's ID
• Card belongs to someone else:
  1. Copy of card in question
  2. Copy of player's ID
  3. Copy of card holder's ID
  4. Completed letter of consent form (Available on MSL website)
• Name Change Documentation
